Cefoxitin Administration Guidelines

Introduction to Cefoxitin Administration

  • The American Thorax Society recommends that cefoxitin must be reconstituted with Sterile Water for Injection and administered intravenously, with doses over 2 grams requiring dilution in sodium chloride 0.9% or glucose 5% for infusion 1

Dosing Recommendations

  • The standard dose for adults is 200 mg/kg/day in three divided doses (maximum 12 g/day) IV, as recommended by the Thorax society 1, 2
  • For serious infections like M. abscessus, the dose can be up to 12 g/day in divided doses, according to the American Journal of Respiratory and Critical Care Medicine 3
  • Typical individual doses are 1-2 grams every 6-8 hours, as suggested by the MMWR Recommendations and Reports and Clinical Infectious Diseases 4, 5
  • For pediatric patients (≥1 month), the standard dose is 150 mg/kg/day in 3-4 divided doses (maximum 12 g/day) IV, as recommended by the Thorax society and Praxis Medical Insights 1, 2, 6

Critical Monitoring Requirements

  • Monitor renal function closely, especially when combining with aminoglycosides or furosemide due to increased nephrotoxicity risk, as advised by the Thorax society and Praxis Medical Insights 1, 2, 6
  • Adjust dose or extend dosing interval if creatinine clearance is reduced, according to Praxis Medical Insights 6
  • Use caution in severe renal impairment or end-stage renal disease, as recommended by the Thorax society 1
  • Perform complete blood count, renal function tests (U&Es), and liver function tests (LFTs) intermittently throughout treatment, as suggested by the Thorax society and Praxis Medical Insights 1, 2
  • Monitor for eosinophilia, leucopenia, and thrombocytopenia, as advised by the Thorax society and Praxis Medical Insights 1, 2

Drug Interactions to Monitor

  • Probenecid reduces renal excretion of cefoxitin, resulting in increased and prolonged serum levels, as reported by the Thorax society and Praxis Medical Insights 1, 2
  • Warfarin: monitor INR closely as cefoxitin increases anticoagulant effect, as recommended by the Thorax society and Praxis Medical Insights 1, 2

Adverse Effects

  • Watch for thrombophlebitis at injection sites, the most common adverse effect, as reported by the Thorax society 1

Cefoxitin Dosage Guidelines for Adult Patients

Standard Adult Dosing by Infection Severity

  • The Surgical Infection Society and IDSA recommend cefoxitin at 2 grams every 6 hours for complicated intra-abdominal infections 7
  • For general adult patients with bacterial infections and normal renal function, administer cefoxitin 1-2 grams intravenously every 6-8 hours, with doses adjusted based on infection severity (up to 12 grams daily for severe infections like gas gangrene) 7

Renal Impairment Dosing Adjustments

  • No specific dose adjustment guidelines for renal impairment are provided with a citation, however, the importance of adjusting doses based on creatinine clearance is emphasized to prevent nephrotoxicity and seizures, although the exact adjustments are not cited 7
