Anaesthetic Management for Patients with Ectopic Kidney
Preoperative Assessment and Planning
- The American Society of Anesthesiologists recommends thorough evaluation of renal function with estimation of glomerular filtration rate (GFR) using standard formulae, and assessment of albumin-to-creatinine ratio (ACR) for patients with ectopic kidneys undergoing major surgery or emergency procedures 1, 2
- Patients with ectopic kidneys should have their medication dosages adjusted according to estimated GFR to prevent acute kidney injury 1, 2
Intraoperative Management
- The American College of Surgeons suggests maintaining adequate renal perfusion pressure with mean arterial pressure between 60-70 mmHg, or >70 mmHg if the patient is hypertensive, and implementing goal-directed fluid therapy to reduce postoperative complications and optimize renal perfusion 1, 2, 3
- Avoiding hypovolemia is crucial to prevent acute kidney injury in patients with abnormally positioned kidneys 1, 2
- Nephrotoxic agents and drugs should be avoided in the perioperative period, and NSAIDs should be used with extreme caution or avoided altogether due to their nephrotoxic effects 1, 2, 4
Postoperative Care
- Close monitoring of renal function with regular assessment of urine output and serum creatinine is essential in the postoperative period, and early detection of acute kidney injury using biomarkers is recommended to guide intervention strategies 1, 2, 3
- Maintaining adequate hydration and avoiding nephrotoxic medications in the postoperative period can help prevent acute kidney injury 1, 2, 4
- Early mobilization and enteral feeding as part of enhanced recovery protocols can also contribute to optimal patient outcomes 3, 7
Special Considerations
- For patients requiring regional anaesthesia, the abnormal position of the kidney may affect the spread of local anaesthetics, and if spinal anaesthesia is planned, consideration of the potential impact on renal perfusion due to sympathetic blockade is necessary 7, 8
- In cases of laparoscopic procedures, monitoring the effects of pneumoperitoneum on renal perfusion, especially in pelvic ectopic kidneys, is crucial to prevent acute kidney injury 3
