Duration of Ivabradine Treatment

Appropriate Duration Based on Indication

  • The American College of Cardiology recommends ivabradine as a reasonable option for ongoing management of patients with symptomatic inappropriate sinus tachycardia (IST) (Class IIa recommendation, Level of Evidence B-R) 1, 2, 3
  • Ivabradine provides persistent clinical benefit in some patients with IST even after discontinuing the drug 2

Dosing and Administration Considerations

  • Initial dosing of ivabradine typically starts at 5 mg twice daily, according to the American College of Cardiology 4
  • Ivabradine can be titrated up to 7.5 mg twice daily based on heart rate response and symptom control, as recommended by the American College of Cardiology 2, 4

Safety Considerations for Long-Term Use

  • Phosphenes (visual brightness phenomena) occur in approximately 3-15% of patients taking ivabradine, but are usually transient, as reported by the American College of Cardiology 2

Monitoring During Long-Term Treatment

  • Regular assessment of heart rate response is recommended for patients taking ivabradine, according to the American College of Cardiology 4
  • Ivabradine should be avoided in patients with severe hepatic impairment, as recommended by the American College of Cardiology 4
  • Ivabradine should be avoided in patients with blood pressure <90/50 mm Hg, as recommended by the American College of Cardiology 4
  • Ivabradine should be avoided in patients with decompensated heart failure, as recommended by the American College of Cardiology 4
