QT Prolongation with Escitalopram

High-Risk Populations Requiring Caution

  • The European Society of Cardiology identifies patients with a history of QT interval prolongation as being at increased risk when prescribed QT-prolonging drugs like escitalopram 1
  • Patients with concurrent antiarrhythmic medications are at increased risk of QT prolongation when taking escitalopram 2
  • The European Heart Journal notes that relevant cardiovascular disease is a risk factor for QT prolongation in patients taking escitalopram 3
  • Bradycardia or conduction abnormalities increase the risk of QT prolongation in patients taking escitalopram 1, 3
  • Thyroid dysfunction is a risk factor for QT prolongation in patients taking escitalopram 1
  • Electrolyte disturbances, such as hypokalemia or hypomagnesemia, increase the risk of QT prolongation in patients taking escitalopram 1, 3

Critical Drug Interactions

  • The European Heart Journal recommends avoiding combinations of QT-prolonging drugs, including escitalopram, to prevent torsades de pointes 3
  • Combining escitalopram with antiarrhythmics (Class IA and III) creates additive risk of QT prolongation 5
  • Macrolide antibiotics and fluoroquinolones should be avoided in combination with escitalopram due to the risk of QT prolongation 4
  • Antipsychotics and other antidepressants should be used with caution in combination with escitalopram due to the risk of QT prolongation 4, 5
  • 5-HT3 antagonists, such as ondansetron and granisetron, should be used with caution in combination with escitalopram due to the risk of QT prolongation 2

Monitoring Recommendations

  • The European Society of Cardiology recommends obtaining baseline ECG before starting escitalopram in patients with cardiac risk factors or on multiple QT-prolonging medications 1
  • Patients should have their electrolyte abnormalities corrected before starting escitalopram, with potassium levels maintained above 4.0 mEq/L and magnesium levels normalized 4
  • ECG should be repeated at 7 days after starting therapy or after any dose increase, particularly in high-risk patients 4
  • Escitalopram should be discontinued immediately if QTc exceeds 500 msec or increases >60 msec from baseline 4
