Treatment for Relationship Obsessive-Compulsive Disorder (ROCD)

Introduction to ROCD

  • The International OCD Foundation suggests that Relationship OCD is a subtype of obsessive-compulsive disorder characterized by intrusive, unwanted thoughts and doubts about one's romantic relationship, repetitive behaviors or mental acts performed to reduce anxiety about the relationship, and significant distress and functional impairment due to time-consuming obsessions and compulsions 3
  • The American Psychological Association notes that common obsessions include doubts about love for partner, partner's attractiveness, or relationship "rightness" 1

First-Line Treatment Approaches

  • The American College of Psychiatrists recommends CBT with ERP as the psychological treatment of choice for all forms of OCD, including ROCD, with ERP involving gradual and prolonged exposure to relationship-focused fears combined with instructions to abstain from compulsive behaviors 2
  • The National Institute of Mental Health suggests that integration of cognitive components with ERP makes treatment less aversive and enhances effectiveness, particularly for patients with poor insight 5
  • The American Psychological Association notes that CBT has larger effect sizes than pharmacological therapy for OCD treatment, with a number needed to treat of 3 for CBT vs. 5 for SSRIs 4

Pharmacotherapy Options

  • The National Alliance on Mental Illness states that serotonin reuptake inhibitors (SRIs) are the first-line pharmacological treatment for OCD, with higher doses typically needed for OCD than for depression 4
  • The American Psychiatric Association recommends that treatment requires careful monitoring of adverse effects, particularly at higher doses, and that medication may be particularly helpful when ROCD is accompanied by comorbid depression 2

Treatment Delivery Options

  • The Journal of Clinical Psychology suggests that individual and group CBT delivered in-person show strong effectiveness, with patient adherence to between-session homework being the strongest predictor of good outcomes 4
  • The International Society for Research on Internet Interventions notes that computer and internet-based treatments can overcome barriers to accessing traditional therapy, with digital interventions containing psychoeducation, cognitive elements, and ERP showing promising results 7

Treatment Algorithm

  • The Academy of Cognitive Therapy recommends evaluating severity of ROCD symptoms using validated measures like Y-BOCS or FOCI, and assessing degree of insight, functional impairment, and presence of comorbidities 8
  • The American College of Psychiatrists suggests beginning with CBT incorporating ERP for mild to moderate ROCD, and considering combined CBT+ERP and SRI medication for severe ROCD or with comorbid depression 2

Common Pitfalls and How to Avoid Them

  • The National Institute of Mental Health notes that addressing shame and stigma through psychoeducation and normalizing ROCD symptoms can help avoid delayed treatment seeking 5
  • The International OCD Foundation recommends educating partners about avoiding reassurance-giving behaviors that reinforce OCD symptoms, and using motivational interviewing techniques to enhance engagement 5
  • The American Psychological Association suggests considering higher doses of medication or more intensive CBT protocols for incomplete response 4
