Treatment of Hypertriglyceridemia

Efficacy and Recommendations

  • The American College of Cardiology recommends gemfibrozil as the first-line treatment for triglyceride levels ≥1500 mg/dL due to its well-established efficacy and safety profile 1, 2
  • Gemfibrozil can reduce triglyceride levels by 44-54%, and is particularly effective in severe hypertriglyceridemia 1, 2
  • The American Diabetes Association suggests that gemfibrozil is recommended specifically as first-line treatment for triglycerides ≥1500 mg/dL, with a established dose of 600 mg twice daily 1, 3
  • In patients with diabetes mellitus, both gemfibrozil and ciprofibrato are effective for hypertriglyceridemia without negative effects on glycemic control 3
  • For patients with high LDL and triglyceride levels (combined hyperlipidemia), high-potency statins are considered the first option, or combination with fibrates 3, 4
  • The combination of any fibrate with statins increases the risk of myopathy, but this risk is higher with gemfibrozil than with fenofibrate/ciprofibrato 4

Treatment Algorithm

  • For triglyceride levels ≥1500 mg/dL, gemfibrozil 600 mg twice daily is the first-line treatment, with ciprofibrato 100 mg once daily as an alternative 1, 2
  • For triglyceride levels between 500-1500 mg/dL, gemfibrozil or ciprofibrato can be used as first-line treatment, with consideration of ciprofibrato in patients with overweight or obesity, and gemfibrozil in patients with high cardiovascular risk 3, 4
  • For combined hyperlipidemia, high-potency statins are the first option, with combination with fibrates if necessary, and preference for ciprofibrato over gemfibrozil when combined with statins 3, 4
