Treatment of Symptomatic Digoxin Toxicity in a Stable Patient

Initial Assessment and Management

  • The American Heart Association recommends monitoring cardiac rhythm, vital signs, and serum potassium levels in patients with symptomatic digoxin toxicity 1, 2

Specific Treatments

  • The American College of Cardiology recommends digoxin-specific antibody fragments (digoxin-Fab) as the first-line treatment for patients with symptomatic digoxin toxicity presenting with life-threatening arrhythmias, significant bradycardia, heart block, or hyperkalemia (>6 mmol/L) 1, 2
  • The European Society of Cardiology reports response rates of 50-90% with digoxin-Fab, with dysrhythmia resolution typically occurring within 30-45 minutes 1

Supportive Measures

  • The American College of Cardiology suggests atropine as a temporizing measure for symptomatic bradyarrhythmias while waiting for digoxin-Fab to take effect 1, 3
  • Temporary cardiac pacing may be considered for severe and unresponsive bradyarrhythmias 1, 3
  • The American Heart Association recommends correcting electrolyte abnormalities, particularly hypokalemia, for ventricular arrhythmias 3
  • Lidocaine or phenytoin may be considered as temporizing measures for ventricular arrhythmias until digoxin-Fab can be administered 1, 4

Ineffective Treatments

  • The American College of Cardiology states that hemodialysis, hemofiltration, hemoperfusion, or plasmapheresis are NOT recommended for digoxin toxicity due to digoxin's large volume of distribution 1, 4

Special Considerations

  • The European Journal of Heart Failure notes that potential adverse effects of digoxin-Fab include exacerbation of heart failure, increased ventricular rate in patients with atrial fibrillation, hypokalemia, and allergic reactions (rare) 2, 3
  • The American College of Cardiology recommends maintaining therapeutic serum concentration between 0.6-1.2 ng/mL after resolution of toxicity 2, 5

Follow-up Monitoring

  • The Journal of the American College of Cardiology suggests monitoring serum digoxin levels, though these may be unreliable after digoxin-Fab administration 3
  • The European Journal of Heart Failure recommends monitoring electrolytes, particularly potassium and magnesium 2
  • The American College of Cardiology advises monitoring renal function, especially in patients with pre-existing renal impairment 5
