Hyponatremia Management

Assessment and Evaluation

  • The severity of hyponatremia can be classified as mild (130-134 mmol/L), moderate (125-129 mmol/L), or severe (<125 mmol/L), with corresponding symptoms such as nausea, headache, weakness, confusion, seizures, or altered mental status 2
  • Determining volume status is crucial, with hypervolemic, euvolemic, or hypovolemic states having different management approaches, and in this case, the patient is likely hypervolemic due to losartan 2

Immediate Interventions

  • For severe symptomatic hyponatremia, administering 3% hypertonic saline with a target correction of 4-6 mEq/L in the first 1-2 hours, not exceeding 10 mEq/L in 24 hours, is recommended 2
  • For moderate or mild hyponatremia, fluid restriction to less than 1L/day is advised 3

Medication Adjustments

  • The American College of Cardiology recommends reducing or discontinuing losartan as the first-line approach, considering alternative antihypertensive medications, and evaluating the need for pantoprazole or reducing its dose as a second-line approach 3, 2
  • Continuing sodium chloride tablets (1g with meals) as currently prescribed is also recommended 3, 2

Monitoring and Prevention

  • Checking serum sodium levels every 4-6 hours during acute correction and monitoring for signs of osmotic demyelination syndrome with rapid correction are essential 2
  • Following serum potassium levels, especially if losartan is discontinued, is also crucial 2
  • The guideline society recommends not exceeding a correction rate of 8-10 mEq/L/day to prevent osmotic demyelination syndrome, with a strength of evidence based on clinical guidelines 4

Special Considerations

  • For persistent hyponatremia despite medication adjustments, considering urea or vaptans (such as tolvaptan) for short-term use in a hospital setting only, with caution in patients with liver disease, is recommended by the guideline society 4
  • Avoiding hypotonic fluids, rapid discontinuation of multiple medications simultaneously, and exercising caution with vaptans in patients with liver disease are also advised by the guideline society 4
