Epley Maneuver Guidelines for Posterior Canal BPPV
Initial Treatment Recommendations
- The American Academy of Otolaryngology-Head and Neck Surgery recommends that patients first receive at least one properly performed Epley maneuver from a trained clinician before attempting home treatment, achieving 80.5% success by day 7 with just 1-3 in-office treatments 1, 2, 3, 4
Success Rates and Treatment Approaches
- Self-Epley with proper instruction achieves a 64% improvement rate 4
Proper Technique and Home Treatment
- Each home Epley maneuver cycle involves holding each position for 20-30 seconds through 5 sequential steps, moving the head and body to reposition displaced otoconia from the posterior semicircular canal back into the vestibule 1, 2, 3
Comparison to Alternative Exercises
- A single Epley maneuver is more than 10 times more effective than a week of Brandt-Daroff exercises performed 3 times daily (OR 12.38; 95% CI 4.32-35.47) 1, 4
- Self-administered Epley maneuvers show 64% improvement compared to only 23% with Brandt-Daroff exercises 4
Treatment Failure and Reassessment
- Canal conversion (switching from one type of BPPV to another, occurring in 6-7% of cases) may occur if home maneuvers fail 4
- Coexisting vestibular conditions requiring different treatment may also lead to treatment failure 4
Post-Treatment Activities
- Post-maneuver restrictions are NOT recommended, and patients can resume normal activities immediately after performing the Epley maneuver 4
