Safety Concerns with Fluvoxamine and Alprazolam Combination

Clinical Risks

  • The FDA has issued a black box warning about combining opioid medications with benzodiazepines due to risks of slowed breathing and death, highlighting the dangers of combining CNS depressants, and this warning is relevant to the combination of fluvoxamine and alprazolam 1
  • The combination of fluvoxamine and alprazolam increases the risk for potentially dangerous respiratory depression, particularly in elderly patients and those with respiratory conditions 1
  • The combination increases the risk for increased risk of falls, particularly in populations that may already be at risk for falls 2

Management Recommendations

  • When the combination of fluvoxamine and alprazolam cannot be avoided, the American College of Cardiology and other guideline societies recommend monitoring closely for signs of excessive sedation, confusion, or coordination problems, and considering alternative anxiety treatments with less interaction potential 2
  • The combination should be used with particular caution in elderly patients and those with respiratory conditions, as they are more susceptible to adverse effects, according to the National Comprehensive Cancer Network 1

Special Precautions

  • The American Heart Association recommends that flumazenil, a benzodiazepine antagonist, has no role in the management of cardiac arrest due to benzodiazepine overdose but may be used to reverse excessive sedation known to be due to benzodiazepines in a patient without known contraindications, as stated in the journal Circulation 3
  • Abrupt discontinuation of either medication may lead to withdrawal symptoms, so tapering is recommended if discontinuation is necessary, according to Praxis Medical Insights 2
