Lovenox Dosing for Acute Ischemic Stroke
Critical Distinction: Treatment vs. Prophylaxis
- The American Heart Association recommends that high-dose Low Molecular Weight Heparin (LMWH), including enoxaparin, should not be used to reduce morbidity, mortality, or early recurrent stroke in acute ischemic stroke patients, due to lack of benefit and increased risk of hemorrhagic complications 1
- The American College of Chest Physicians suggests that anticoagulation for stroke treatment has no proven benefit due to increased hemorrhagic complications, and therefore is not recommended for therapeutic goals 1
- No specific stroke subgroup benefits from therapeutic-dose enoxaparin, according to the American Heart Association 1
VTE Prophylaxis
- The American Heart Association recommends enoxaparin 40 mg subcutaneously once daily for Deep Vein Thrombosis (DVT) prophylaxis in immobilized acute ischemic stroke patients 2, 3
- Immediate antiplatelet therapy, such as aspirin 160-325 mg loading dose, should be initiated after hemorrhage is excluded, and continued at 81-325 mg daily for secondary prevention, as recommended by the American College of Chest Physicians 3
Acute Stroke Management
- The American College of Chest Physicians recommends against using therapeutic-dose enoxaparin for stroke treatment, as it increases bleeding without proven benefit 1, 5
- The American Heart Association suggests that aspirin therapy should not be delayed in favor of anticoagulation, and should be started within 48 hours unless contraindicated 3
