Psoriasis Treatment Guidelines

Initial Assessment and Disease Severity Classification

  • The American Academy of Dermatology recommends assessing disease severity using both objective and subjective measures, with mild psoriasis defined as <3% body surface area, moderate psoriasis as 3-10% body surface area, and severe psoriasis as >10% body surface area 2
  • The National Institute for Health and Care Excellence (NICE) guidelines state that body surface area alone is insufficient for determining disease severity, and that psoriasis affecting palms/soles, face, genitals, or scalp with <5% BSA may still warrant systemic therapy due to significant quality of life impact 3, 4, 1

Treatment Algorithm for Mild Psoriasis

  • The American College of Dermatology recommends combination therapy with topical corticosteroids (class 2-5) and vitamin D analogs (calcipotriene) for maximum 4 weeks, then transitioning to weekend-only corticosteroid application with weekday vitamin D analog use for maintenance 1, 2
  • The European Academy of Dermatology and Venereology guidelines suggest that combination therapy is superior to monotherapy, with topical corticosteroids and vitamin D analogs providing a synergistic effect and greater efficacy than either agent alone 1, 2

Treatment Algorithm for Moderate-to-Severe Psoriasis

  • The Group for Research and Assessment of Psoriasis and Psoriatic Arthritis (GRAPPA) guidelines recommend TNF inhibitors as superior to conventional synthetic DMARDs (csDMARDs) as first-line therapy, particularly in early disease 7
  • The American Academy of Dermatology guidelines suggest that biologic agents (TNF inhibitors, IL-12/23 inhibitors, IL-17 inhibitors, IL-23 inhibitors) should be considered together with traditional systemics based on patient characteristics, not as last resort 7

Systemic Agents

  • The National Institute for Health and Care Excellence (NICE) guidelines recommend methotrexate as a first-line systemic option, with a starting dose of 15 mg weekly initially, maximum 25-30 mg weekly 8, 2
  • The European League Against Rheumatism (EULAR) guidelines suggest that cyclosporine is an effective treatment option, with a dose of 2.5-5 mg/kg daily, but limited to 1 year use in US (2 years in UK) due to glomerulosclerosis risk 8, 2
  • The American College of Rheumatology guidelines recommend acitretin as a first-line option for pustular and erythrodermic psoriasis, with a dose of 25-50 mg daily 3, 4, 6

Biologic Therapy

  • The Group for Research and Assessment of Psoriasis and Psoriatic Arthritis (GRAPPA) guidelines recommend TNF inhibitors (etanercept, adalimumab, infliximab) as effective treatments for moderate-to-severe psoriasis, with superior efficacy to csDMARDs as first-line therapy in early disease 7
  • The European Academy of Dermatology and Venereology guidelines suggest that ustekinumab (IL-12/23 inhibitor) shows 67% clearance in palmoplantar pustular psoriasis at 90mg dose 6

Special Clinical Scenarios

  • The American Academy of Dermatology guidelines recommend acitretin 25-50 mg daily as first-line treatment for pustular psoriasis, with ustekinumab as an alternative option for moderate-to-severe cases 3, 4, 2, 6
  • The European League Against Rheumatism (EULAR) guidelines suggest that cyclosporine, infliximab, or acitretin are alternative treatment options for erythrodermic psoriasis 6

Critical Medications to Avoid

  • The National Institute for Health and Care Excellence (NICE) guidelines recommend that systemic corticosteroids should never be used as monotherapy in psoriasis, as they can cause disease flare during or after taper and precipitate pustular or erythrodermic psoriasis 3, 4, 1, 2, 6
  • The American College of Dermatology guidelines suggest that lithium, chloroquine, and mepacrine are associated with severe, potentially fatal psoriasis deterioration, and should be avoided 1
