Thrombocytopenia with Elevated MPV Diagnosis and Management
Introduction to Thrombocytopenia
- The American Society of Hematology suggests that heparin-induced thrombocytopenia (HIT) must be considered if the patient has received any heparin products within the past 3 months, as this carries significant thrombotic risk 1
- Vaccine-induced immune thrombocytopenia and thrombosis (VITT) should be considered if symptoms began 5-30 days post COVID-19 vaccination, particularly if accompanied by thrombosis or very elevated D-dimer 2, 3
Diagnostic Evaluation
- A peripheral blood smear examination is recommended to confirm true thrombocytopenia, assess platelet morphology, and exclude pseudothrombocytopenia or other hematologic abnormalities 1
- The Centers for Disease Control and Prevention (CDC) guidelines suggest that infection-related thrombocytopenia, including tickborne rickettsial diseases, can present with thrombocytopenia and elevated transaminases 4
Clinical Management
- The American College of Cardiology recommends assessing for bleeding symptoms, including petechiae, purpura, mucosal bleeding, or other hemorrhagic manifestations that would increase urgency 1
- Evaluating for thrombotic complications is necessary, particularly if there is recent heparin or vaccine exposure, as some hyperdestructive conditions paradoxically increase thrombotic risk 1, 2, 3
