Rivastigmine Dosing and Side Effects

Dosing Regimen

  • The American Academy of Family Physicians recommends starting rivastigmine at 1.5 mg twice daily with meals, and increasing by 1.5 mg twice daily every 4 weeks as tolerated, targeting a maintenance dose of 6-12 mg per day, with higher doses providing superior efficacy 1
  • The initial dosing of 1.5 mg twice daily is generally well tolerated and minimizes gastrointestinal side effects 1

Side Effect Profile

  • The American Academy of Family Physicians reports that the most frequent side effects are gastrointestinal and include nausea, vomiting, diarrhea, weight loss, abdominal pain, headaches, dizziness, fatigue, malaise, anxiety, and agitation 1, 5
  • Nausea, vomiting, and diarrhea are the most commonly reported adverse events, and can be minimized by taking rivastigmine with food 1, 2

Special Considerations

  • The American Academy of Sleep Medicine notes that patients with age-related changes in drug metabolism or progression of neurologic disease may require dose adjustments over time 3
  • The American Academy of Family Physicians recommends allowing 6-12 months to assess full therapeutic response, and using physician global assessment, caregiver reports, neuropsychologic testing, and behavioral/functional changes to evaluate response 1, 2, 4

Management of Side Effects

  • Taking all doses with meals can reduce gastrointestinal symptoms 1, 2
  • Discontinuation criteria include development of side effects that do not resolve, poor adherence, and continued deterioration at the pretreatment rate after 6-12 months 1

Important Clinical Considerations

  • The American Academy of Family Physicians notes that interacting drugs include aminoglycosides and procainamide 1, 5
  • Rivastigmine has pharmacokinetic advantages, including pseudo-irreversible cholinesterase inhibition lasting up to 10 hours, and low protein binding 6
