Antiplatelet Management for Surgical Debridement
Introduction to Antiplatelet Therapy
- The American College of Chest Physicians recommends stopping clopidogrel 5 days before surgical debridement, prasugrel 7 days before, and aspirin 7-10 days before in patients with low cardiovascular risk, but continuing aspirin in moderate-to-high cardiovascular risk patients 1, 2, 3
Risk-Stratified Approach to Antiplatelet Management
- For patients with moderate-to-high cardiovascular risk, the American College of Chest Physicians recommends continuing aspirin throughout the perioperative period rather than stopping it 1, 4
- For patients with low cardiovascular risk, the American College of Chest Physicians recommends stopping aspirin 7-10 days before surgery 1, 4
- The American College of Chest Physicians recommends discontinuing clopidogrel 5 days before surgery 2, 3, 5, 6, 7
- The American College of Chest Physicians recommends discontinuing prasugrel 7 days before surgery 5, 6, 7
Special Considerations for High-Risk Patients
- For patients with bare-metal stents, the American College of Chest Physicians recommends deferring elective surgery for at least 6 weeks after placement 1, 2, 3
- For patients with drug-eluting stents, the American College of Chest Physicians recommends deferring elective surgery for at least 6 months after placement 1, 2, 3
- If surgery cannot be delayed, the American College of Chest Physicians recommends continuing dual antiplatelet therapy perioperatively rather than stopping 7-10 days before surgery 2, 3, 4
Resumption of Antiplatelet Therapy
- The American College of Chest Physicians recommends resuming antiplatelet therapy within 12-24 hours after surgery when adequate hemostasis is achieved 1, 3, 9
- For clopidogrel at maintenance dose (75 mg/day), maximal platelet inhibition takes 5-10 days 8, 9
- A loading dose (300-600 mg) of clopidogrel achieves maximal inhibition within 12-15 hours if rapid effect is needed 8, 9
