ESMO Guideline‑Recommended Management of Very High‑Risk M0 Prostate Cancer with N1 Disease

Definition of Very High‑Risk Disease

  • Patients with any of the following meet the European Society for Medical Oncology (ESMO) definition of very high‑risk M0 prostate cancer: positive lymph nodes (N1), Gleason score ≥ 8, or extracapsular extension (T3). This patient exhibits all three criteria【1】.

Core Treatment Recommendation

  • ESMO recommends a radiotherapy‑based regimen for N1 disease, consisting of external beam radiotherapy (EBRT) to the prostate and pelvis combined with long‑course androgen deprivation therapy (ADT) for 24–36 months plus abiraterone plus prednisone for 24 months【1】【2】【3】.

Evidence of Survival Benefit (STAMPEDE Trial)

  • In the updated STAMPEDE trial (randomized Phase III, Level I evidence), the triplet regimen (EBRT + ADT + abiraterone) produced a hazard ratio for overall survival of 0.63 (95 % CI 0.48–0.82; P = 0.0005) compared with standard therapy【1】【3】.
  • The same analysis showed a hazard ratio for metastasis‑free survival of 0.54 (95 % CI 0.43–0.68) with the intensified regimen【1】【3】.

Regulatory Status and Strength of Recommendation

  • Although abiraterone is not formally approved by the FDA or EMA for non‑metastatic (M0) disease, ESMO assigns it a Level I, Grade B recommendation for use in very high‑risk patients【1】【2】【3】.

Patient Selection and Treatment Intensity

  • For elderly patients who are medically fit, the full intensified regimen (EBRT + ADT + abiraterone) should be offered; ADT alone is reserved for those unable to tolerate treatment intensification【1】【2】.

Alternative When Full Regimen Is Not Tolerated

  • If the triplet therapy cannot be administered, EBRT combined with long‑course ADT (24–36 months) alone remains an acceptable step‑down approach and provides superior outcomes to ADT alone【1】.
