Preoperative Imaging in Elective Abdominal Aortic Aneurysm Surgery

Indications for Preoperative Angiography

  • The American College of Radiology recommends CT angiography (CTA) or MR angiography (MRA) as the primary preoperative imaging modalities for AAA evaluation, with catheter arteriography having very limited utility 2, 3
  • Catheter arteriography is now primarily reserved for specific clinical scenarios where other imaging modalities are contraindicated or additional information is needed, such as in patients with a history of claudication 1, 4
  • The European Heart Journal suggests that angiography in patients with claudication helps identify the extent of peripheral arterial disease and the need for concomitant revascularization procedures 5

Preoperative Imaging Modalities

  • The American College of Radiology recommends multidetector CT (MDCT) or CTA as the optimal choice for pre-intervention studies in elective AAA repair 2, 3
  • MRA may be substituted if CT cannot be performed, such as in patients with contrast allergy 1, 3

Contraindications and Pitfalls

  • Relying on angiography alone for AAA size assessment is inappropriate; CTA or ultrasound provides more accurate diameter measurements 2, 4
  • Delaying intervention for angiography in symptomatic or ruptured AAA can increase mortality 5
