Antibiotic Resistance and Treatment Regimens

Introduction to Resistance

  • The use of Bismuth quadruple therapy or concomitant non-bismuth quadruple therapy avoids the issue of clarithromycin resistance, which is increasingly common worldwide, as recommended by the American Gastroenterological Association 1
  • The American Gastroenterological Association recommends Bismuth quadruple therapy for 14 days as the preferred first-line treatment regimen for H. pylori infection when antibiotic susceptibility is unknown, with an eradication rate of approximately 85% 1

Diagnosis and Confirmation of Eradication

  • The Urea Breath Test (UBT) is the gold standard non-invasive test, with sensitivity of 95% and specificity of 90%, or a monoclonal stool antigen test as an alternative with equivalent accuracy, for confirmation of eradication 2
  • A 13C urea breath test or stool antigen test is preferred for primary care due to high sensitivity and specificity (>90%) 3, 4
  • The European Society of Gastrointestinal Endoscopy recommends waiting at least 4 weeks after completing antibiotic therapy before testing for H. pylori eradication to ensure accurate results 5

Treatment Regimens and Eradication Rates

Regimen Duration Eradication Rate
Bismuth quadruple therapy 14 days 85%
Concomitant non-bismuth quadruple therapy 14 days 80%
Standard triple therapy 14 days 85%

6, 1, 7, 8, 4, 5

  • If first-line therapy fails, the choice of second-line therapy should avoid antibiotics previously used, particularly if treatment failure is suspected to be due to antibiotic resistance 1
  • The American Gastroenterological Association recommends Rifabutin-based triple therapy or High-dose dual therapy for patients who have failed two previous treatment attempts 9

Dietary Management and Lifestyle Modifications

  • A diet low in acidic, spicy, and fatty foods, along with regular consumption of fruits and vegetables rich in vitamin C, is recommended as first-line dietary management for patients with epigastric pain and a history of H. pylori infection, as suggested by the American Gastroenterological Association 4
  • Patients should avoid acidic foods, spicy foods, and fatty foods to prevent irritation of the gastric lining and delay gastric emptying, according to the American Gastroenterological Association 4
  • Regular aerobic exercise is strongly recommended by the British Society of Gastroenterology for patients with persistent symptoms 3

Special Considerations and Precautions

  • Increasing resistance to clarithromycin, levofloxacin, and metronidazole is a major cause of treatment failure, and local antibiotic resistance patterns should guide therapy selection when available 1
  • Patients with true penicillin allergy can be treated with Bismuth quadruple therapy with tetracycline instead of amoxicillin or alternative regimens using levofloxacin or bismuth-based therapies, according to the American Gastroenterological Association 6, 9
  • Most medications used for H. pylori treatment are compatible with breastfeeding, as stated in the Gut journal 10
  • The American Gastroenterological Association recommends avoiding invasive diagnostic procedures, such as endoscopy, during pregnancy unless absolutely necessary for clinical decision making, and if needed, perform preferably in the second trimester 11

Surveillance and Monitoring

  • Surveillance endoscopy should be performed every 3 years in patients with risk factors for gastric cancer or atrophic gastritis, with a strength of evidence of high quality, as suggested by the American College of Gastroenterology 12
  • Patients with age ≥ 55 or presence of alarm symptoms should be referred for prompt endoscopy with H. pylori testing, as recommended by the American College of Gastroenterology 13, 14
