Current PAH Treatment Standards

• The American College of Cardiology and European Heart Society recommend intravenous prostacyclin analogues, such as epoprostenol and treprostinil, for high-risk PAH patients, with a strength of evidence level of I (high-quality evidence) 1
• Risk stratification guides therapy selection, with high-risk patients (WHO FC IV, 6MWD <165m, cardiac index <2.0 L/min/m², RAP >14 mmHg) recommended to receive intravenous prostacyclin therapy, based on moderate-quality evidence from the European Heart Journal 2, 3
• Right heart catheterization is mandatory to confirm diagnosis, assess severity, and guide therapeutic decisions before initiating any advanced PAH therapy, with a strong recommendation based on high-quality evidence from the European Respiratory Journal and European Heart Journal 4, 5, 1
