Neuromuscular Blockade with Cisatracurium

Introduction to Cisatracurium Dosage

  • The recommended perfusion dose of cisatracurium for adult patients requiring neuromuscular blockade is 3 mcg/kg/min initially, followed by maintenance at 1-2 mcg/kg/min, titrated to achieve one or two twitches on train-of-four monitoring 1
  • Initial bolus dose: 0.1-0.2 mg/kg IV bolus to establish neuromuscular blockade 1
  • Metabolism independent of organ function: Cisatracurium undergoes Hofmann elimination and ester hydrolysis, making it suitable for patients with renal or hepatic dysfunction 1
  • Predictable recovery: Recovery typically occurs within 34-85 minutes after discontinuation, independent of organ function 1

Monitoring and Adjustment

  • Neuromuscular monitoring: Use TOF monitoring to guide dosing 1, 2
  • Target: Adjust dose to achieve one or two twitches on TOF monitoring 1
  • Assessment frequency: Regular clinical and TOF assessment throughout therapy 1

Pharmacological Properties

  • Hemodynamic stability: Cisatracurium produces minimal cardiovascular effects and virtually no histamine release compared to other neuromuscular blocking agents 2

Safety Considerations

  • Inadequate sedation: Always ensure adequate sedation and analgesia before initiating neuromuscular blockade 1
  • Insufficient monitoring: Failure to use TOF monitoring can lead to excessive or inadequate blockade 1, 2
