Meropenem Dosing Recommendations in Adults

Standard Dosing for Specific Infections

  • Complicated intra‑abdominal infections in adults with normal renal function: 1 g IV every 8 hours. This regimen is supported by multiple infectious disease studies. 1, 2, 3
  • Hospital‑acquired or ventilator‑associated pneumonia in adults with normal renal function: 1 g IV every 8 hours. Evidence from clinical trials underpins this dosing. 1, 3

Pharmacodynamic Optimization in Critically Ill Patients

  • Extended or continuous infusion strategies: For critically ill patients, infections with high minimum inhibitory concentrations (MIC > 2 mg/L), or septic shock, infusing meropenem over 3 hours (extended) or via continuous infusion after a loading dose is recommended to maximize the percentage of the dosing interval that the free drug concentration exceeds the MIC (≥40 % fT>MIC), which correlates with bacterial eradication. 4
