Treatment of Bilateral Otitis Media
Diagnosis and Treatment
- The American Academy of Pediatrics recommends antibiotic therapy with amoxicillin as the first-line treatment for bilateral otitis media, particularly in children younger than 2 years of age 1, 2
- Accurate diagnosis is essential and should be based on the presence of moderate to severe bulging of the tympanic membrane or new onset of otorrhea not due to acute otitis externa 2, 3
- For children 6 months to 2 years of age with bilateral AOM, antibiotic therapy is recommended 1, 4
- The American Academy of Pediatrics strongly recommends antibiotic therapy for bilateral AOM in children under 2 years of age due to higher risk of treatment failure with observation alone 1, 2
Antibiotic Selection
- Amoxicillin (80-90 mg/kg/day in 2 divided doses) is the recommended first-line antibiotic due to its effectiveness against common pathogens, safety, low cost, acceptable taste, and narrow microbiologic spectrum 5, 2, 6
- For penicillin-allergic patients with non-type I hypersensitivity reactions, cefdinir, cefuroxime, or cefpodoxime may be used 7, 6
Duration of Therapy and Follow-up
- The standard course of antibiotic therapy is typically 10 days for children under 2 years of age 2, 6
- For children 2 years and older with mild to moderate disease, a 5-7 day course may be sufficient 2, 6
Clinical Evidence Supporting Treatment Decisions
- Studies show that antibiotics are most beneficial in children younger than 2 years with bilateral AOM, with a number needed to treat (NNT) of 3 for clinical success 1, 8
- In a randomized controlled trial, clinical failure rates by day 10-12 in children with bilateral AOM were 23% with amoxicillin-clavulanate versus 60% with placebo (NNT = 3) 8
