Osteoporosis Prevention Guidelines
Primary Recommendation: Bisphosphonates
- The American College of Physicians strongly recommends bisphosphonates as initial pharmacologic treatment for preventing osteoporosis in postmenopausal women and high-risk individuals, based on high-certainty evidence showing they reduce hip fractures by 50% and vertebral fractures by 47-56% over 3 years, with a preferred initial choice of alendronate 70 mg once weekly or risedronate 35 mg once weekly 1, 2
- Bisphosphonates have a superior balance of efficacy, safety, patient preferences, and cost compared to all other drug classes, making them the recommended first-line medication 1, 3
Essential Concurrent Supplementation
- The American College of Physicians recommends that all patients starting bisphosphonates must receive calcium 1,200 mg daily and vitamin D 800 IU daily, as pharmacologic therapy is significantly less effective without adequate supplementation, with a target serum vitamin D level of ≥20 ng/mL 2, 3, 4
Treatment Duration and Monitoring Strategy
- Initial treatment duration with bisphosphonates is 5 years, after which fracture risk should be reassessed to determine if continued therapy is warranted, with patients at low risk for fracture considered for drug discontinuation after 3 to 5 years of use 1, 2, 3
- Bone density should not be monitored during the initial 5-year treatment period, as this provides no clinical benefit 2, 4
Special Consideration: Osteopenia (Low Bone Mass)
- For postmenopausal women with osteopenia, the American College of Physicians suggests that bisphosphonate use should be based on individualized fracture risk assessment, with zoledronate potentially reducing the risk of clinical vertebral fractures in this population 5
Safety Profile
- High-certainty evidence shows no difference in serious adverse events between bisphosphonates and placebo in randomized controlled trials at 3+ years, but rare serious adverse effects include osteonecrosis of the jaw and atypical femoral fractures 1, 2, 3, 4
Second-Line Option
- Denosumab 60 mg subcutaneously every 6 months is recommended as second-line therapy for patients with contraindications to or intolerance of bisphosphonates, with a critical warning to never discontinue denosumab abruptly without transitioning to bisphosphonate therapy 1, 3, 4
