Rabies Vaccination Guidelines
Introduction to Rabies Vaccination
- The Centers for Disease Control and Prevention recommends a complete 4-dose vaccine series for previously unvaccinated individuals on days 0, 3, 7, and 14, along with Human Rabies Immunoglobulin (HRIG) at 20 IU/kg on day 0, to provide both immediate passive immunity and long-term active immunity 1, 2, 3, 4
- The World Health Organization suggests that previously vaccinated individuals receive only 2 vaccine doses, one immediately (day 0) and one on day 3, with no HRIG administered, as it would inhibit the rapid anamnestic immune response 1, 2, 5
Vaccination Protocol
- For previously unvaccinated persons, a 4-dose vaccine series is administered on days 0, 3, 7, and 14, along with HRIG at 20 IU/kg on day 0 1, 2
- For previously vaccinated persons, only 2 vaccine doses are needed: one immediately (day 0) and one on day 3, with no HRIG administered 1, 2, 5
- The immune system of previously vaccinated individuals develops a rapid antibody response following booster vaccination 5
Special Populations
- Immunocompromised individuals may require a 5-dose schedule (days 0, 3, 7, 14, and 28) with HRIG, and serologic testing 7-14 days after the last dose should be performed to ensure adequate antibody response 2, 7
- The American College of Physicians recommends that immunocompromised individuals receive additional doses of the vaccine, as they may have suboptimal immune responses 5, 7
Critical Clinical Pitfalls to Avoid
- The Centers for Disease Control and Prevention advises against administering HRIG to previously vaccinated individuals, as it can impair the immune response 1, 2, 5
- The World Health Organization recommends immediate thorough wound cleansing with soap and water, two vaccine doses (days 0 and 3), and appropriate wound management and tetanus prophylaxis as indicated, for previously vaccinated persons 1, 2, 4, 5, 6, 7
