Timing of Losartan Administration for Hypertension Management

Evidence-Based Recommendation

  • The European Society of Cardiology recommends that current evidence does not show benefit of diurnal timing of blood pressure-lowering drug administration on major cardiovascular outcomes, and patients should take medications at the most convenient time of day to improve adherence, at the same time each day in a consistent setting 1
  • The European Society of Cardiology explicitly states that the timing of losartan administration does not affect major cardiovascular outcomes, and a consistent daily routine is more important than the specific time of day 1

Supporting Evidence from Other Guidelines

  • The American Diabetes Association notes that while there is an association between absence of nocturnal blood pressure dipping and cardiovascular disease incidence, a meta-analysis found only a small benefit of evening versus morning dosing on blood pressure control with no data on clinical effects (morbidity/mortality outcomes) 2, 3

Important Caveats

  • The key issue is achieving adequate total daily dosing—studies show 150mg daily is superior to 50mg daily for heart failure outcomes, according to the European Society of Cardiology and European Journal of Heart Failure 4, 5, 6
  • Ensuring adequate total daily dose (typically 50-100mg for hypertension, up to 150mg for heart failure if tolerated) is crucial, as recommended by the European Journal of Heart Failure and European Heart Journal 4, 5, 6
