Antiplatelet and Anticoagulation Therapy for Elderly NSTEMI Patients

Introduction to Antiplatelet and Anticoagulation Therapy

  • The American College of Cardiology recommends administering aspirin 150-300 mg loading dose, then 75-100 mg daily, clopidogrel 300-600 mg loading dose, then 75 mg daily, and enoxaparin 1 mg/kg subcutaneously every 12 hours, with critical dose reduction to 1 mg/kg once daily if creatinine clearance is below 30 mL/min, for elderly patients with NSTEMI 1, 2

Aspirin Dosing Guidelines

  • The European Heart Journal suggests that aspirin should be given to all patients without contraindications as soon as possible after presentation, with a loading dose of 150-300 mg of non-enteric-coated, chewable aspirin immediately upon presentation 3, 4
  • The American College of Cardiology recommends continuing 75-100 mg of aspirin daily indefinitely 1, 2, 3

Clopidogrel Dosing Recommendations

  • The American College of Cardiology recommends a clopidogrel loading dose of 300-600 mg orally at presentation, and a maintenance dose of 75 mg once daily 1, 2
  • The European Heart Journal suggests continuing clopidogrel for at least 12 months unless contraindications or excessive bleeding risk exist 3, 4

Enoxaparin Dosing and Renal Impairment Adjustments

  • The American College of Cardiology recommends an initial enoxaparin dose of 1 mg/kg subcutaneously every 12 hours for the duration of hospitalization or until PCI, with an optional 30 mg IV bolus in selected patients 1, 2, 4, 6
  • For patients with creatinine clearance below 30 mL/min, the dose should be reduced to 1 mg/kg subcutaneously once daily 1, 2, 4, 6

Timing of Enoxaparin Relative to PCI

  • If PCI occurs after enoxaparin initiation, no additional enoxaparin is needed if it has been less than 8 hours since the last dose 1, 2, 6
  • If it has been 8-12 hours since the last dose, an IV bolus of 0.3 mg/kg should be administered at the time of PCI 1, 2, 6

Critical Safety Warnings and Monitoring

  • The American College of Cardiology warns against administering UFH to patients already receiving enoxaparin, as this significantly increases bleeding complications 6
  • The European Heart Journal recommends discontinuing parenteral anticoagulation immediately after PCI, and continuing dual antiplatelet therapy for a minimum of 12 months post-NSTEMI 3, 4

Common Pitfalls to Avoid in Enoxaparin Administration

  • Failure to calculate creatinine clearance before dosing is a common error leading to enoxaparin overdosing in elderly patients 6
  • Using standard twice-daily dosing in patients with CrCl <30 mL/min causes dangerous accumulation 1, 2
