Thyroid Function Testing in Psychiatric Evaluation

Rationale for Medical Clearance

  • Medical disorders, such as thyroid disease, must be excluded before attributing symptoms solely to psychiatric or substance use causes, as they can present with mood swings, irritability, anxiety, and depression, according to the American Academy of Pediatrics and American College of Emergency Physicians 1
  • The American Academy of Pediatrics and American College of Emergency Physicians support that routine laboratory testing should be medically indicated based on clinical suspicion from history and physical examination, not performed as protocol 1
  • Thyroid storm or severe hypothyroidism, which can present with mood swings, irritability, and unkempt appearance, require urgent medical intervention 1

Substance Use Disorder Considerations

  • Depression and anxiety are significantly more common in patients with substance use disorders, particularly substance dependence, as indicated by the American Family Physician guidelines 3, 5
  • Brief counseling using motivational interviewing approach is recommended for substance abuse, along with close follow-up and ongoing assessment, according to the American Family Physician 2, 3
  • Referral to specialty treatment for intensive behavioral therapy and consideration of pharmacotherapy options are recommended for substance dependence, as suggested by the American Family Physician 2, 3, 5

Treatment Planning

  • Combined cognitive behavioral therapy and pharmacotherapy shows small to moderate benefits for substance use disorders with psychiatric comorbidities, as reported in the JAMA Network Open 6
  • The American Family Physician guidelines recommend treating underlying depression and anxiety that are not being treated, as they significantly increase the risk for substance use disorders and poor outcomes 3, 5

Common Pitfalls to Avoid

  • Do not assume psychiatric symptoms are solely due to substance use or primary psychiatric illness without ruling out medical causes, such as thyroid disease, electrolyte abnormalities, and other metabolic disorders, as cautioned by the Pediatrics journal 1
  • Do not order routine laboratory panels without clinical indication, as this wastes resources and rarely changes management, according to the Pediatrics journal 1
