Denosumab Administration in Patients with Renal Impairment

Dosing Considerations

  • The National Comprehensive Cancer Network recommends that denosumab does not require dose adjustment for patients with impaired renal function, including those with eGFR <30 mL/min/1.73 m² or on dialysis, which is an advantage over bisphosphonates like zoledronic acid that require dose reduction when creatinine clearance is 30-60 mL/min and must be held when <30 mL/min 1

Pre-Treatment Requirements

  • The American College of Clinical Oncology and the National Comprehensive Cancer Network recommend that baseline dental evaluation is necessary for patients at high risk of osteonecrosis of the jaw (ONJ), particularly those with planned invasive dental procedures 2, 1

Safety Monitoring

  • The National Comprehensive Cancer Network reports that hypocalcemia risk is significantly elevated with denosumab (13%) compared to zoledronic acid (6%) in patients with bone metastases 1
  • The American Society of Clinical Oncology and the National Comprehensive Cancer Network state that ONJ incidence is approximately 1-2% with denosumab 60 mg every 6 months, similar to bisphosphonates 2, 1
  • The National Comprehensive Cancer Network recommends maintaining optimal oral hygiene throughout treatment, deferring denosumab therapy until complete healing is confirmed by the dentist if invasive dental surgery is necessary, and avoiding invasive dental procedures during active treatment when possible 1
