Nitrate Administration in Hypotensive Patients Without Angina

Absolute Contraindications to Nitrate Administration

  • The American College of Cardiology recommends that nitrates are contraindicated in patients with systolic blood pressure less than 90 mm Hg or ≥30 mm Hg below baseline, due to the risk of profound hypotension without therapeutic benefit 1, 2, 3
  • Nitrates are also contraindicated in patients with severe bradycardia (<50 beats per minute), tachycardia (>100 beats per minute) in the absence of symptomatic heart failure, right ventricular infarction, and recent phosphodiesterase inhibitor use (sildenafil within 24 hours, tadalafil within 48 hours) 1, 2, 3

Mechanism of Action and Therapeutic Indications

  • The primary therapeutic indication for nitrates is relief of myocardial ischemia through venous dilation reducing cardiac preload and ventricular wall tension, coronary artery vasodilation improving blood flow to ischemic regions, and redistribution of coronary blood flow and promotion of collateral flow 3, 4

Clinical Context: When Nitrates Are Appropriate

  • Nitrates should only be considered when there is active myocardial ischemia with angina despite adequate beta-blocker therapy, heart failure with pulmonary congestion, or hypertension in the setting of acute coronary syndrome 1, 3
  • Even in these appropriate clinical scenarios, nitrates must be withheld if systolic blood pressure is <90 mm Hg or has dropped ≥30 mm Hg from baseline 1, 2, 3

Alternative Management Approach

  • For hypotensive patients without angina, the American College of Cardiology recommends focusing on identifying and treating the underlying cause of hypotension, providing volume resuscitation if appropriate, considering vasopressor support if needed, and monitoring closely for development of ischemia 1, 2
