Use of Prednisone in Patients with Chronic Kidney Disease

Indications for Prednisone in CKD

  • The Kidney International guideline recommends prednisone as a first-line therapy for primary FSGS with nephrotic syndrome, starting at 1 mg/kg/day (maximum 80 mg) for at least 4 weeks if tolerated 1, 2
  • For membranous nephropathy with moderate-to-high risk of progression, the Kidney International guideline recommends glucocorticoids combined with cyclophosphamide or rituximab 3
  • In minimal change disease, the Kidney International guideline recommends prednisone 1 mg/kg/day (max 80 mg) or 2 mg/kg alternate-day (max 120 mg) as the standard initial treatment, even with reduced kidney function 1
  • The American Journal of Kidney Diseases suggests that in children with mesangiocapillary glomerulonephritis (MCGN), alternate-day prednisone 40 mg/m² improved renal survival to 61% at 130 months versus 12% with placebo 4

Contraindications for Prednisone in CKD

  • The American Journal of Kidney Diseases states that advanced CKD with severe tubulointerstitial fibrosis, small kidney size, or chronic inactive disease is an absolute contraindication for immunosuppression, as it will not help and only causes harm 4
  • The American Journal of Kidney Diseases also states that IgA-dominant postinfectious glomerulonephritis is an absolute contraindication for steroid use, as it must be distinguished from IgA nephropathy to avoid inappropriate steroid use 4
  • The Diabetes Care guideline recommends that diabetic kidney disease without active glomerular inflammation is a contraindication for steroid use, as steroids worsen glycemic control without renal benefit 5

Monitoring Requirements in CKD

  • The Praxis Medical Insights guideline recommends assessing current blood pressure control and eGFR before initiating prednisone 7
  • The Praxis Medical Insights guideline also recommends ensuring blood pressure is at target before starting prednisone, and intensifying antihypertensive therapy if necessary 7
  • The Diabetes Care guideline recommends monitoring for hyperkalemia, especially if the patient is on ACE inhibitors, ARBs, or mineralocorticoid receptor antagonists 5

Dosing Adjustments and Precautions

  • The Kidney International guideline recommends using the lowest effective dose of prednisone for the shortest duration to minimize complications 3
  • The Kidney International guideline also recommends halving cyclophosphamide doses in patients with eGFR <50 mL/min receiving cyclophosphamide combinations 3

Alternative Approaches

  • The Kidney International guideline recommends calcineurin inhibitors (cyclosporine 3-5 mg/kg/day or tacrolimus 0.05-0.1 mg/kg/day) as effective alternatives for FSGS and minimal change disease 1, 2
  • The Kidney International guideline also recommends rituximab as an alternative for membranous nephropathy, which can avoid steroid toxicity 3
  • The Diabetes Care guideline recommends focusing on SGLT2 inhibitors (if eGFR ≥20), ACE inhibitors/ARBs, and nonsteroidal MRAs (if eGFR ≥25) for CKD progression without active glomerular disease 5
