Treatment of Osteoporosis in Patients with GFR 30

Assessment Before Treatment

  • The American College of Rheumatology recommends evaluating patients with osteoporosis and a GFR of 30 mL/min for chronic kidney disease-mineral and bone disorder (CKD-MBD) through measurement of serum calcium, phosphate, PTH, and alkaline phosphatase activity 1, 3
  • Patients should be assessed for renal osteodystrophy and hyperparathyroidism before initiating treatment, using tests such as bone-specific alkaline phosphatase, intact PTH, and possibly bone biopsy 1, 4
  • Fracture risk should be assessed with appropriate tools, taking into account factors such as glucocorticoid use, and using tools like FRAX with adjustments as necessary 5, 6

Treatment Recommendations

  • The American College of Rheumatology conditionally recommends denosumab as a first-line treatment for patients with GFR 30 mL/min, due to its lack of requirement for dose adjustment for renal function, with careful monitoring of calcium levels 1, 2
  • PTH/PTHrP analogs, such as teriparatide, may be considered as alternative options for patients with GFR 30 mL/min, as they do not require renal dose adjustment 1, 2

Calcium and Vitamin D Supplementation

  • The American College of Rheumatology recommends optimizing calcium and vitamin D intake as foundational treatment for patients with osteoporosis and GFR 30 mL/min, with a target serum 25(OH)D level >30 ng/mL 2, 7
  • Biologically active vitamin D, such as calcitriol, paricalcitol, or doxercalciferol, may be required instead of vitamin D3 or D2 for patients with GFR <30 mL/min 1

Monitoring

  • Serum calcium and phosphorus levels should be monitored regularly, at least monthly for the first 3 months of therapy, then every 3 months, and PTH levels should be monitored at least every 3 months for the first 6 months, then every 3 months thereafter 7

Special Considerations

  • For patients on glucocorticoids, the treatment approach should consider both the osteoporosis and the glucocorticoid-induced bone loss, according to the American College of Rheumatology 1, 8
  • Solid organ transplant recipients with GFR 30 mL/min should undergo expert evaluation for CKD-MBD, as conditionally recommended by the American College of Rheumatology 2
