FOBT Screening Frequency for Colorectal Cancer

  • The American College of Physicians, U.S. Preventive Services Task Force, American Cancer Society, and U.S. Multi-Society Task Force recommend annual FOBT as the appropriate screening interval for average-risk adults aged 50 and older 1, 2, 5, 6
  • The American Medical Association supports annual FOBT beginning at age 50 for average-risk adults 3, 4

Critical Implementation Requirements

  • The Canadian Task Force on Preventive Health Care recommends FOBT every 2 years for adults aged 50-74 years, although annual testing remains the predominant recommendation globally 2, 5
  • Home-based specimen collection is required for FOBT, with single-panel FOBT performed in the office using a stool sample from digital rectal examination not recommended due to low sensitivity 6
  • High-sensitivity gFOBT should be used rather than older, lower-sensitivity versions 7

Duration of Screening

  • Annual FOBT should be continued through age 75 years for average-risk individuals in good health 2, 5, 7
  • For ages 76-85 years, screening decisions should be individualized based on life expectancy, prior screening history, and overall health status 2, 5, 8
  • Screening should be discontinued at age 85 years or when life expectancy is less than 10 years 5, 7, 8

Important Caveats

  • All positive FOBT results require follow-up colonoscopy to rule out colorectal cancer or advanced neoplasia 2, 5, 6
  • Commitment to annual testing is essential, as one-time or sporadic FOBT testing has very limited sensitivity and makes stool testing a poor screening choice 6
