Imiquimod Treatment for Periungual Warts

Application and Efficacy

  • The Centers for Disease Control and Prevention (CDC) guidelines recommend applying imiquimod 5% cream with a clean finger at bedtime, three times per week, for up to 16 weeks, with a washout period of 6-10 hours after application, for the treatment of periungual warts 1, 2
  • The treatment should be continued for up to 16 weeks, though many patients may clear by 8-10 weeks, with the provider demonstrating proper application technique at the first visit 1
  • Washing the treatment area with mild soap and water 6-10 hours after application is recommended, typically in the morning 1, 2
  • The American Academy of Dermatology and CDC guidelines suggest that imiquimod offers a non-destructive alternative to conventional therapies, which can cause significant inflammation, pain, and pigment changes 1, 2

Safety and Precautions

  • Pregnancy safety for imiquimod is not established, and patients should be counseled about the possibility of hypopigmentation, particularly in darker-skinned individuals 1, 2
  • Earlier treatment yields better results, with patients having shorter disease duration responding better to imiquimod treatment 2
  • Consider switching treatment if no substantial improvement is seen after 8 weeks 2
