Emergency Management of Phenol Poisoning
Initial Management
- The American Heart Association recommends calling emergency services or poison control center immediately while initiating treatment for phenol poisoning, and removing all contaminated clothing and jewelry to prevent continued exposure 1, 2
- The American College of Cardiology suggests immediate decontamination with polyethylene glycol (PEG) is the preferred treatment, followed by thorough washing with soap and water, and removing all contaminated clothing and jewelry to prevent continued exposure while avoiding self-contamination 3, 4
Route-Specific Decontamination
Eye Exposure
- The American Academy of Ophthalmology recommends flushing eyes immediately with copious amounts of tepid water for at least 15 minutes in cases of phenol eye exposure 3
Treatment for Phenol Poisoning
Initial Management
- The American Heart Association recommends contacting poison control center or emergency services immediately upon suspicion of phenol exposure, and advises not to administer anything by mouth for any poisoning ingestion unless advised by poison control center or emergency medical personnel 5, 6
Ingestion
- The American Heart Association advises against inducing vomiting and administering alcohol or oral mineral oil as they may increase absorption, and considers immediate administration of olive oil and activated charcoal by small bore nasogastric tube if advised by poison control center, and does not recommend administering activated charcoal unless advised by poison control center or emergency medical personnel 5, 6
Management of Phenol Poisoning
Immediate Priorities and Decontamination
- Contact your regional poison control center immediately for specialized toxicological consultation, as phenol poisoning requires expert guidance that most clinicians use infrequently, according to the American Heart Association 7
- Establish and maintain vital functions with standard life support measures, as recommended by the American Heart Association 7
