Diffuse Hair Loss Treatment Guidelines

Diagnostic Approach

  • The diagnostic approach for diffuse hair loss should include laboratory testing, such as fungal culture, skin biopsy, serology for lupus erythematosus, thyroid stimulating hormone (TSH) to rule out thyroid disease, and vitamin D, zinc, and folate levels to identify nutritional deficiencies, as recommended by Praxis Medical Insights 1

Treatment Options by Type

  • For telogen effluvium, spontaneous remission occurs in up to 80% of cases with short duration (<1 year), according to the British Journal of Dermatology 2
  • Nutritional supplementation, such as vitamin D, zinc, and folate, may be beneficial if deficiencies are identified in patients with telogen effluvium, as suggested by Praxis Medical Insights 1
  • For diffuse alopecia areata, intralesional corticosteroids (triamcinolone acetonide 5-10 mg/mL) are a treatment option, as reported by the British Journal of Dermatology 3
  • Oral prednisolone (starting at 40 mg daily with 6-week tapering course) may show >25% regrowth in 30-47% of patients with diffuse alopecia areata, according to the British Journal of Dermatology 2

Special Considerations

  • The psychological impact of hair loss should be addressed as part of management, as emphasized by Praxis Medical Insights 4
