Alprazolam for Panic Attacks: Clinical Use and Guidelines

Introduction to Alprazolam Treatment

  • The World Health Organization recommends psychological treatment based on Cognitive Behavioral Therapy (CBT) principles as the preferred treatment for people concerned about prior panic attacks, rather than benzodiazepines, for individuals with anxiety complaints in the absence of a diagnosed panic disorder 1, 2, 3, 4

Appropriate Use of Alprazolam

  • Alprazolam should be reserved for patients who have failed or cannot access CBT-based psychological interventions, according to the World Health Organization guidelines 1, 4
  • The World Health Organization suggests that benzodiazepines, such as alprazolam, should not be used for initial treatment of individuals with anxiety complaints in the absence of a diagnosed panic disorder 4

Discontinuation and Safety Considerations

  • Discontinuation of alprazolam is particularly difficult and associated with serious rebound and withdrawal symptoms, emphasizing the need for gradual dose reduction when discontinuing therapy, as recommended by the National Comprehensive Cancer Network 5, 6
