Treatment Duration and Monitoring for Major Depressive Disorder

Standard Treatment Guidelines

  • The American College of Physicians, as reported in the Annals of Internal Medicine, recommends continuing treatment for 4-9 months after satisfactory response in first-episode major depressive disorder, and considering years to lifelong therapy for patients with 2 or more depressive episodes 1, 2
  • The treatment guidelines suggest assessing patient status within 1-2 weeks of initiation for therapeutic response, adverse effects, and emergence of suicidal thoughts or behaviors, with a strength of evidence based on clinical trials 1, 2
  • Close monitoring is advised during the first 1-2 months when suicide risk is highest, with regular follow-up appointments to adjust treatment as needed 1, 2
  • Modification of treatment is recommended if inadequate response is observed after 6-8 weeks, with consideration of alternative therapies or dose adjustments 1, 2
