Empiric Therapy for MRSA and Pseudomonas Infections

Introduction to Empiric Coverage

  • The Infectious Diseases Society of America recommends empiric coverage for MRSA and Pseudomonas based on specific risk factors, local prevalence patterns, and severity of illness, with MRSA coverage indicated when prevalence exceeds 10-20% and dual Pseudomonas coverage when risk factors are present or in high-mortality situations 1, 2

MRSA Coverage Indications

  • Initiate empiric MRSA coverage in patients with hospital-acquired pneumonia (HAP) when prior intravenous antibiotic use within 90 days, hospitalization in a unit where >20% of S. aureus isolates are methicillin-resistant, unknown local MRSA prevalence, or high risk for mortality are present 2, 3
  • Include MRSA coverage in patients with ventilator-associated pneumonia (VAP) when risk factors for antimicrobial resistance, treatment in units where >10-20% of S. aureus isolates are methicillin-resistant, or treatment in units where MRSA prevalence is unknown are present 1, 4

Pseudomonas Coverage Indications

  • Single antipseudomonal agent is appropriate for most HAP patients without specific risk factors, while dual antipseudomonal coverage is indicated when prior intravenous antibiotic use within 90 days, high risk for mortality, or structural lung disease are present 2, 5
  • Risk factors for MDR Pseudomonas in VAP include prior intravenous antibiotic use within 90 days, septic shock at time of VAP, ARDS preceding VAP, five or more days of hospitalization prior to VAP, and acute renal replacement therapy prior to VAP onset 1, 4

Empiric Antibiotic Selection

  • First-line options for MRSA coverage include vancomycin and linezolid, with dosing recommendations of 15 mg/kg IV q8-12h for vancomycin and 600 mg IV q12h for linezolid 2, 5
  • Single agent options for Pseudomonas coverage include piperacillin-tazobactam, cefepime or ceftazidime, levofloxacin, imipenem, and meropenem, with specific dosing recommendations for each 2, 5
  • For dual coverage, add one of the following to a β-lactam: ciprofloxacin, aminoglycoside, or avoid using two β-lactams together 2, 5

Implementation Considerations

  • All hospitals should regularly generate and disseminate local antibiograms, and empiric regimens should be based on local pathogen distribution and susceptibility patterns 2, 3
  • Clinical reassessment within 48-72 hours is essential to ensure appropriate response, and if no improvement occurs within 72 hours, consider additional cultures and possible therapy adjustment 6
