Tetanus Vaccination Guidelines for Adults Undergoing Surgery
Vaccination Decision Algorithm
- The Centers for Disease Control and Prevention recommends administering a tetanus booster (Tdap preferred if not previously received, otherwise Td) if the patient's last tetanus vaccination was ≥10 years ago 1, 2
- Attempt to document the patient's complete tetanus vaccination history, including whether they completed a primary series (3 doses) and the date of their most recent booster 3, 4
- Patients with military service since 1941 can be considered to have received at least one dose, though completion of the primary series cannot be assumed 5
- If vaccination history is unknown or uncertain, consider the patient to have had no previous tetanus toxoid doses 3, 4
Key Clinical Considerations
- No tetanus vaccine is needed if the patient was vaccinated within the past 10 years, and tetanus immune globulin (TIG) is not indicated for clean surgical wounds in fully immunized patients 1, 2
- For patients with documented complete primary series (≥3 doses), administer tetanus toxoid-containing vaccine if the last dose was ≥10 years ago 3, 4, 1
- Protective antibodies develop rapidly in persons who have previously received at least two doses of tetanus toxoid 3, 4
Contraindications and Precautions
- History of anaphylaxis to any vaccine component is an absolute contraindication 3, 10
- History of encephalopathy within 7 days of previous pertussis-containing vaccine is an absolute contraindication for Tdap (use Td instead) 10
