Iron Dosing for CKD Stage 3

Initial Assessment and Oral Iron Therapy

  • The American Journal of Kidney Diseases recommends oral iron at 200 mg elemental iron daily, divided into 2-3 doses, as the first-line approach for CKD stage 3 patients, with a preference for ferrous sulfate or ferrous fumarate formulations due to their cost-effectiveness and high elemental iron content 1
  • Before initiating iron therapy, it is essential to exclude active infection, as recommended by Kidney International and Praxis Medical Insights 2, 3

Intravenous Iron Therapy

  • The Kidney International and Praxis Medical Insights guidelines suggest that intravenous iron should be strongly considered when oral iron fails, causes intolerable side effects, or when rapid iron repletion is needed, with iron sucrose being the preferred formulation due to its safety profile 2, 3
  • Iron dextran requires a test dose due to the risk of anaphylaxis, and its use should be cautious, as recommended by Praxis Medical Insights 4

Safety Monitoring and Target Iron Parameters

  • The American Journal of Kidney Diseases and Praxis Medical Insights recommend mandatory 60-minute post-infusion observation with resuscitation equipment immediately available during IV iron administration, as well as monitoring for hypotension and arthralgias/myalgias 3, 5, 6
  • The target iron parameters include TSAT >20%, ferritin 100-500 ng/mL, and avoiding ferritin levels >500 ng/mL, as recommended by Praxis Medical Insights 3

Critical Safety Considerations

  • Active infection is an absolute contraindication for iron therapy, and ferritin levels >500 ng/mL should be avoided, as recommended by Kidney International and Praxis Medical Insights 2, 3, 4
