CT Scan Timing and Expectations After Thrombolysis for Ischemic Stroke

Timing of Follow-up Imaging

  • A repeat CT or MRI brain scan should be performed 24 hours after the initial stroke event in patients treated with thrombolysis, even if the patient is clinically stable, as recommended by the American Heart Association 1, 2
  • For patients who experience clinical deterioration, emergency CT scanning should be performed immediately, according to the American College of Cardiology 3
  • Neurological assessments should be performed every 15 minutes during the tPA infusion, every 30 minutes for the next 6 hours, then hourly until 24 hours after treatment, as suggested by the American Heart Association 3

What to Expect on Follow-up Imaging

  • Resolution or stability of initial ischemic changes can be expected on follow-up imaging, as noted by the American Stroke Association 2
  • No evidence of hemorrhagic transformation is a desirable outcome on follow-up imaging, as recommended by the American Heart Association 1
  • Infarct progression is an abnormal finding that can occur on follow-up imaging, as reported by the American Stroke Association 2

Clinical Implications of Follow-up Imaging

  • The 24-hour follow-up CT scan is required before starting anticoagulants or antiplatelet agents for secondary stroke prevention, as recommended by the American College of Cardiology 3
  • Delaying antiplatelet therapy in cases of hemorrhagic transformation may be necessary, as suggested by the American Heart Association 3

Pitfalls and Caveats

  • Relying solely on clinical examination without follow-up imaging may miss asymptomatic hemorrhagic transformation, as noted by the American Stroke Association 2
  • Delay in obtaining follow-up imaging may postpone initiation of secondary stroke prevention measures, as reported by the American College of Cardiology 3

Repeat Cranial CT Scan in Ischemic Stroke

Emergency Repeat CT Indications

  • The International Journal of Stroke recommends immediate repeat CT scanning for any neurological deterioration, including decline in level of consciousness or change in Canadian Neurological Scale score by ≥1 point, worsening NIHSS score by ≥4 points from baseline 4

Monitoring Framework Prior to Repeat Imaging

  • The International Journal of Stroke suggests hourly neurological assessments as the minimum standard for acute stroke patients, with more frequent monitoring for those receiving thrombolysis or with high-risk features, and blood pressure should be monitored at least hourly 4
  • Patients with uncontrolled hypertension, diabetes, or hyperlipidemia should have a lower threshold for obtaining emergency repeat imaging when subtle clinical changes occur, according to the International Journal of Stroke 4
