Approach to Fever with Calf Pain
Essential Initial Workup
- The American College of Physicians recommends two sets of blood cultures before antibiotics to identify bacteremia from rickettsial diseases, leptospirosis, or other systemic infections 1
- A complete blood count with differential is recommended to look for leukocytosis with left shift, thrombocytopenia, and anemia, which may suggest rickettsial disease or leptospirosis 1
- A comprehensive metabolic panel, including liver function tests and renal function, is recommended to evaluate for elevated aminotransferases and BUN/creatinine, which may indicate leptospirosis or rickettsial infection 1
Critical History Elements
- The Centers for Disease Control and Prevention recommends obtaining a recent travel history within the past year to tropical or subtropical regions, as rickettsial diseases and leptospirosis are geographically specific 2, 1
- Exact locations visited, dates of travel, and timing of symptom onset relative to return should be documented 1
- Outdoor activities with potential tick exposure should be assessed, as this may increase the risk of rickettsial diseases 2
Geographic and Exposure-Specific Differential
- Rickettsial diseases, such as murine typhus, scrub typhus, and RMSF, may present with fever, headache, calf pain/tenderness, and rash, and are more common in certain geographic areas 2
- The Weil-Felix reaction and Rickettsia-specific IFT may be helpful in diagnosing rickettsial diseases 2
Empirical Treatment Algorithm
- The Infectious Diseases Society of America recommends initiating empirical antibiotics immediately in systemically ill patients with suspected rickettsial disease, without delaying treatment until diagnostic confirmation 1
- Doxycycline 100 mg PO/IV twice daily is recommended for suspected rickettsial disease 2
Consultation Triggers
- Immediate infectious disease/tropical medicine consultation is recommended for critically ill patients with tropical exposure, undiagnosed fever after initial workup in returned travelers, and suspected rickettsial disease or leptospirosis requiring species-specific treatment guidance 1
