Alternative Sleep Medications to Lunesta (Eszopiclone)

First-Line Non-Pharmacological Treatment

  • The American College of Physicians recommends cognitive behavioral therapy for insomnia (CBT-I) as the initial treatment approach for chronic insomnia before considering any medication, with demonstrated effectiveness in improving sleep outcomes including reduced sleep onset latency, wake after sleep onset, and improved sleep efficiency 1

Pharmacological Alternatives

  • Zolpidem is effective for both sleep onset and sleep maintenance insomnia with a half-life of approximately 2.4 hours, and the FDA has approved it for short-term use (4-5 weeks) 1
  • The American Academy of Sleep Medicine reports that zolpidem improves subjective sleep latency by approximately 19.55 minutes compared to placebo 3
  • Suvorexant has moderate-quality evidence showing it improves treatment response and sleep outcomes, and is particularly effective for sleep maintenance, reducing wake after sleep onset by 16-28 minutes compared to placebo 1
  • Doxepin provides strong evidence for sleep maintenance, with low-dose doxepin (3-6 mg) improving Insomnia Severity Index scores and sleep outcomes, and reducing wake after sleep onset by 22-23 minutes 1

Special Considerations

  • For elderly patients, the American Academy of Sleep Medicine recommends considering lower doses (zolpidem 5 mg, suvorexant 5 mg, doxepin 3 mg) due to increased sensitivity to medication side effects 3
  • The American College of Physicians notes that all hypnotic medications carry risks of adverse effects including daytime impairment, "sleep driving," behavioral abnormalities, and potential for dependence 1

Duration of Treatment

  • The FDA has approved pharmacologic therapy for short-term use (4-5 weeks), and patients should not continue using these medications for extended periods without reevaluation 1

Algorithm for Selection

  • The American Academy of Sleep Medicine suggests starting with CBT-I as first-line therapy, and considering adding pharmacotherapy based on primary complaint, patient age and comorbidities, previous medication responses, and side effect profiles and contraindications 1
