Evaluation of Megaloblastic Crisis

Initial Laboratory Assessment

  • The American Society of Hematology recommends immediately measuring serum vitamin B12, serum folate, and red blood cell folate levels to identify the most common causes of megaloblastic anemia in patients with macrocytic anemia 1, 2
  • A complete blood count with peripheral smear is essential to confirm macrocytic anemia, evaluate for oval macrocytes, hypersegmented neutrophils, and assess for pancytopenia, with a mean corpuscular volume (MCV) >100 fL 3
  • Serum folate and red blood cell folate levels should be measured, with RBC folate ≥340 nmol/L indicating adequate folate status 5, 2
  • Peripheral blood smear examination is crucial for dysplasia and blast enumeration, and should be performed in all patients with suspected megaloblastic anemia 1

Exclude Other Causes of Macrocytosis

  • The National Comprehensive Cancer Network recommends serum iron, total iron binding capacity, and ferritin tests to exclude iron deficiency or identify concurrent deficiencies in patients with macrocytosis 6, 7
  • Lactate dehydrogenase (LDH) should be measured, as elevated levels indicate ineffective erythropoiesis in megaloblastic anemia 7
  • Bilirubin and haptoglobin tests are necessary to assess for hemolysis in patients with macrocytosis 7, 1
  • Thyroid function tests should be performed to exclude hypothyroidism as a cause of macrocytosis, as recommended by the American Thyroid Association 2

Determine Underlying Etiology

  • The American Gastroenterological Association recommends intrinsic factor antibodies to diagnose pernicious anemia in patients with vitamin B12 deficiency 4
  • Medication review is essential to identify drugs interfering with B12 absorption, such as metformin, H2 receptor antagonists, and proton pump inhibitors, as recommended by the American College of Gastroenterology 4
  • Dietary history should be taken to assess for strict vegetarian/vegan diet, which provides no B12, and to identify patients at risk of B12 deficiency 4

Myelodysplastic Syndrome Evaluation

  • The International Working Group for the Diagnosis, Standardization of Response Criteria, Treatment Outcomes, and Reporting Standards for Therapeutic Trials in Acute Myeloid Leukemia recommends cytogenetic analysis to detect clonal chromosomal abnormalities in patients with suspected myelodysplastic syndrome 6, 7, 1
  • Flow cytometry should be performed to assess CD34+ cells and detect abnormal antigenic expression in patients with suspected myelodysplastic syndrome 6, 1

Critical Pitfalls to Avoid

  • The American Society of Hematology warns against treating with folic acid before excluding or treating B12 deficiency, as folic acid doses >0.1 mg daily may produce hematologic remission while allowing irreversible neurologic damage to progress 2
  • The National Comprehensive Cancer Network recommends against relying solely on serum B12 levels, as functional deficiency may exist with "normal" values, requiring MMA or homocysteine confirmation 4

Special Population Considerations

  • The American College of Gastroenterology recommends that post-bariatric surgery patients be screened for B12 malabsorption, as they are at high risk of deficiency 4
  • The American Geriatrics Society recommends that elderly patients (≥60 years) be screened for metabolic B12 deficiency, which has a prevalence of 18-25% in this population 4

Laboratory Interpretation Caveats

  • The Clinical and Laboratory Standards Institute recommends that inflammatory conditions be considered when interpreting ferritin levels, as they can elevate ferritin despite iron deficiency 2
  • The American Society of Hematology recommends that a wide RDW be used to identify coexisting iron deficiency in patients with macrocytosis 2
