Allopurinol Therapy Considerations

Patient Risk Factors

  • The American Journal of Kidney Diseases recommends considering HLA-B*58:01 testing before initiating allopurinol therapy in high-risk ethnic groups, such as Han-Chinese, Korean, and Thai populations, due to increased risk of hypersensitivity reactions 1
  • Patients with renal impairment require dose reduction, with a recommended reduction of at least 50% in patients with renal insufficiency, as suggested by the Journal of Clinical Oncology 4, 5

Drug Interactions and Contraindications

  • The Journal of Clinical Oncology advises a 65-75% dose reduction of purine-based chemotherapeutic agents, such as 6-mercaptopurine and azathioprine, when used concomitantly with allopurinol, due to reduced clearance 4
  • Allopurinol is contraindicated with cyclophosphamide and other cytotoxic agents due to increased bone marrow suppression, as reported by the Journal of Clinical Oncology 2, 3

Dosing and Monitoring

  • The Annals of the Rheumatic Diseases suggests starting with low doses (100 mg daily) and titrating gradually to minimize the risk of acute gout attacks 6
  • The Journal of Clinical Oncology recommends reducing the dose of allopurinol by at least 50% in patients with renal insufficiency, and considering dose adjustments when used concomitantly with certain medications, such as colchicine or statins 4, 5
