Resistant Hypertension Management

Direct Recommendations

  • The European Society of Cardiology recommends optimizing chlorthalidone dose to maximum (25 mg daily) and adding hydralazine 25-50 mg twice daily as the sixth agent, while addressing medication adherence and secondary causes 1, 2
  • The European Society of Cardiology suggests adding hydralazine 25 mg twice daily, titrating to 50 mg twice daily as tolerated, due to its direct arterial vasodilator effect with a complementary mechanism to the existing five-drug regimen 1, 2

Medication Selection

  • Hydralazine is particularly effective at higher doses in truly resistant hypertension, and the 2024 ESC Guidelines list it as an appropriate add-on after spironolactone, beta-blocker, and standard triple therapy fail 1, 2, 3
  • Doxazosin 1 mg daily at bedtime, titrating to 4-8 mg daily, provides additional vasodilation through alpha-1 blockade, but start low due to first-dose orthostatic hypotension risk 1, 2, 4
  • Clonidine 0.1 mg twice daily, titrating to 0.3 mg twice daily, is a centrally acting alpha-2 agonist that reduces sympathetic outflow, but has a higher adverse effect profile 1, 2, 3

Lifestyle Intensification

  • Sodium restriction to <2 g/day (87 mmol/day) is paramount in resistant hypertension, and can reduce BP by 10-20 mmHg in volume-dependent resistant hypertension 1, 2, 3
  • Restricting alcohol to <100 g/week (<14 units/week men, <8 units/week women) and maintaining aerobic exercise ≥150 min/week moderate intensity are also recommended 1

Blood Pressure Targets

  • The target is systolic BP 120-129 mmHg and diastolic <80 mmHg, with a minimum target of <140/90 mmHg as the first objective 1, 2

Critical Pitfalls to Avoid

  • Do not combine lisinopril with an ARB due to increased hyperkalemia and acute kidney injury risk without benefit 3
  • Do not add minoxidil yet, as it is reserved as an absolute last resort due to severe fluid retention and hirsutism requiring loop diuretic and beta-blocker 3
  • Monitor potassium and creatinine closely (every 1-2 weeks initially) when on spironolactone + ACE inhibitor combination 1, 2

Renal Denervation

  • Consider referral for catheter-based renal denervation if BP remains uncontrolled after optimizing all six medications, which may provide an additional 5-10 mmHg reduction in truly resistant cases 1, 2

Management of Uncontrolled Hypertension on Maximum-Dose Telmisartan

Stepwise Approach

  • The American Heart Association recommends adding a calcium channel blocker (amlodipine 5-10 mg) or a thiazide/thiazide-like diuretic to the telmisartan 160 mg regimen, with preference for adding amlodipine as the next step 5, 6
  • Add amlodipine 5 mg daily to the existing telmisartan 160 mg regimen as the preferred next step for non-Black patients 5, 6, 7
  • Add a thiazide or thiazide-like diuretic (e.g., hydrochlorothiazide 12.5-25 mg or chlorthalidone) if calcium channel blockers are contraindicated or not tolerated 5, 6
  • Thiazide diuretics demonstrate significant additive antihypertensive benefit when combined with ARBs 8

Resistant Hypertension Management

  • The European Society of Cardiology recommends adding low-dose spironolactone (12.5-25 mg daily) as the preferred fourth agent for resistant hypertension 9, 8
  • Spironolactone provides an additional 25/12 mmHg reduction in systolic/diastolic blood pressure when added to multidrug regimens 8
  • Check serum potassium and creatinine before initiating spironolactone (potassium must be <5.0 mmol/L, creatinine <250 µmol/L) 9

Critical Considerations

  • Verify adherence to the current telmisartan regimen before escalating therapy 5, 6
  • Verify blood pressure measurements with home blood pressure monitoring or 24-hour ambulatory monitoring (target <135/85 mmHg for home BP, <130/80 mmHg for 24-hour ambulatory BP) 5, 6
  • Reinforce lifestyle modifications, particularly sodium restriction, which is critical in resistant hypertension 9

Common Pitfalls to Avoid

  • Avoid combining two ARBs or an ARB with an ACE inhibitor—this increases adverse events without improving outcomes 9
  • Do not add spironolactone without first ensuring adequate diuretic therapy is in place 9

Blood Pressure Targets

  • Target blood pressure <130/80 mmHg for most patients 5, 6
  • Achieve target within 3 months of treatment intensification 5, 6
