Diagnostic Approach for Tuberculosis in Dialysis Patients
Initial Diagnostic Testing
- The American College of Chest Physicians recommends that XpertMTB/RIF testing replace sputum microscopy as the initial diagnostic test for patients with suspected pulmonary TB, with a pooled sensitivity of 89% and specificity of 99% 1, 2, 3
- XpertMTB/RIF provides rapid results (within hours) and simultaneously detects rifampin resistance, which is crucial for guiding appropriate treatment 3
- Sputum culture results take weeks to months, causing dangerous delays in diagnosis and treatment initiation, and microscopy (smear) has only 60% sensitivity, missing many active TB cases 1
Diagnostic Algorithm
- The CHEST guidelines recommend immediately obtaining sputum for GeneXpert MTB/RIF testing, and simultaneously sending sputum for mycobacterial culture and drug susceptibility testing 1, 2, 3
- A chest X-ray should be obtained when feasible 1, 2
- Empiric treatment initiation should be considered while awaiting results, given high-risk immunocompromised status and classic symptoms 4, 5
