Direct Switching from Sertraline to Citalopram

  • A direct, wash‑out‑free transition from sertraline to citalopram is acceptable; start citalopram at a low dose the day after stopping sertraline and monitor closely for 24–48 h for serotonin syndrome. 1
  • Both agents are short‑half‑life SSRIs, allowing immediate substitution without the 14‑day gap required for MAOIs. 2
  • When two non‑MAOI serotonergic drugs are overlapped, the second drug should be initiated at a low dose, titrated slowly, and patients should be observed especially during the first 24–48 h. 3

Initial and Titration Dosing (Pediatrics)

  • Begin citalopram at 10 mg once daily (sub‑therapeutic “test” dose). 5
  • This low starting dose reduces the risk of serotonin syndrome during the transition. 6
  • If tolerated, increase to 20 mg once daily after 1–2 weeks. 4
  • A dose of 20 mg/day is generally the effective therapeutic dose for most patients. 5

Monitoring for Serotonin Syndrome (AACAP)

  • Observe patients for the first 24–48 h for:
    • Mental status changes – confusion, agitation, anxiety. 6
    • Neuromuscular hyperactivity – tremor, clonus, hyperreflexia, rigidity. 3
    • Autonomic hyperactivity – hypertension, tachycardia, arrhythmia, diaphoresis, vomiting. 3
  • Advanced manifestations (fever, seizures, loss of consciousness) warrant immediate hospitalization. 2

Monitoring for Sertraline Discontinuation Syndrome (AACAP)

  • During the first week after sertraline cessation, watch for:
    • Dizziness, vertigo, imbalance. 3
    • Fatigue, generalized malaise, muscle aches. 3
    • Nausea, vomiting, diarrhea. 3
    • Paresthesias or other sensory disturbances. 1
    • Anxiety, irritability, agitation. 1

Safety Considerations

Drug‑Interaction Profile (AACAP)

  • Citalopram has a lower potential for pharmacokinetic interactions because it exerts minimal inhibition of CYP450 enzymes compared with other SSRIs. 4
  • It carries a reduced risk of discontinuation syndrome relative to paroxetine. 4

Cardiac Risks (AACAP & European Heart Journal)

  • Doses > 40 mg/day are associated with QT‑interval prolongation, torsades de pointes, ventricular tachycardia, and sudden death; therefore, do not exceed 40 mg/day. 1
  • Avoid citalopram in patients with known long‑QT syndrome. 4
  • Use caution when co‑prescribing other QT‑prolonging agents. 2
  • Obtain a baseline ECG in patients with cardiovascular risk factors before initiating citalopram. 7

Practical Pitfalls to Avoid (AACAP)

  • Do not start citalopram at the full therapeutic dose (20 mg) immediately; begin with 10 mg. 5
  • Do not forgo close monitoring during the first 48 h after the switch. 1

Overlap Strategy for Severe Discontinuation History (AACAP)

  • For patients with a history of severe SSRI discontinuation, a brief 2‑day overlap may be used:
    • Days 1‑2: Continue full‑dose sertraline while adding citalopram 10 mg daily. 3
    • Day 3: Discontinue sertraline; continue citalopram 10 mg.
  • This approach mitigates discontinuation symptoms but requires even more intensive monitoring for serotonin syndrome. 1
