Management of Mitral Valve Replacement with Bioprosthesis and LAA Clip Ligation
Immediate Postoperative Anticoagulation Strategy
- The European Society of Cardiology recommends initiating anticoagulation early postoperatively using intravenous unfractionated heparin with aPTT monitoring to 1.5-2.0 times control until therapeutic INR is achieved with warfarin (target INR 2.0-3.0) 1
- The American College of Cardiology suggests that if low molecular weight heparin is used instead, anti-factor Xa monitoring is essential, especially in patients with renal dysfunction or obesity where dosing is challenging 1
- Oral warfarin should be begun within 24-48 hours postoperatively, overlapping with heparin until INR is therapeutic for 2 consecutive days 1
Duration and Intensity of Anticoagulation
- The European Society of Cardiology recommends that all patients with bioprosthetic MVR require warfarin anticoagulation for a minimum of 3 months postoperatively (INR 2.0-3.0), regardless of whether LAA ligation was performed 1, 2, 4
- The American College of Cardiology suggests that if atrial fibrillation is present, continue lifelong anticoagulation with warfarin (INR 2.0-3.0) 1, 4
- The American College of Cardiology notes that the LAA clip ligation does NOT eliminate the need for anticoagulation in AF patients, as incomplete occlusion occurs in approximately 50% of cases and thrombus can form elsewhere in the left atrium 5, 2
Postoperative Monitoring Protocol
- The American College of Cardiology recommends baseline transthoracic echocardiography should be performed before hospital discharge to establish a reference for future comparisons and assess bioprosthetic valve function and gradients 4
- The European Society of Cardiology suggests follow-up echocardiography at 3 months to reassess valve function, detect new-onset or recurrent AF, and guide anticoagulation decisions 1
- The European Society of Cardiology recommends INR monitoring, checking INR every 2-3 days initially until stable therapeutic range achieved, then weekly for 1 month, then monthly if consistently therapeutic, with a target INR 2.0-3.0 for bioprosthetic MVR 1
Special Consideration for the Bioprosthesis
- The European Society of Cardiology notes that structural valve deterioration typically begins 7-10 years postoperatively (earlier in younger patients) 1
- The American College of Cardiology recommends annual echocardiographic surveillance starting at 5 years post-implantation to detect early degeneration 6
