Treatment of Complicated Urinary Tract Infections in Young Males
Introduction to Complicated UTIs
- The European Association of Urology recommends treating a 20-year-old male with his first UTI as a complicated UTI, requiring 7-14 days of antibiotics, with mandatory urine culture and susceptibility testing before initiating empirical therapy 1, 2
- UTIs in males are automatically classified as complicated UTIs regardless of other factors, according to the 2024 European Association of Urology guidelines 1, 2
Diagnostic Considerations
- Obtain urine culture and susceptibility testing before starting antibiotics, as this is mandatory for all complicated UTIs and cannot be skipped 1, 3
- Assess for systemic symptoms, such as fever, rigors, flank pain, and costovertebral angle tenderness 1
- Evaluate for potential prostatitis, including perineal pain, obstructive voiding symptoms, and tender prostate on examination 2, 3
Antibiotic Selection
- For patients with systemic symptoms, use combination IV therapy, such as amoxicillin plus an aminoglycoside, or a second-generation cephalosporin plus an aminoglycoside 1, 3
- For patients without systemic symptoms, ciprofloxacin may be used only if local fluoroquinolone resistance rate is <10%, the entire treatment can be given orally, the patient does not require hospitalization, and the patient has not used fluoroquinolones in the last 6 months 1, 2, 3
Treatment Duration
- The recommended duration for men when prostatitis cannot be excluded is 14 days 2, 3
- 7 days may be considered only if the patient is hemodynamically stable, has been afebrile for at least 48 hours, prostatitis has been definitively excluded, and there are relative contraindications to the antibiotic requiring shorter duration 3
Follow-Up Actions
- Adjust the antibiotic to the narrowest spectrum agent effective against the isolated uropathogen once culture and susceptibility results return 1, 3
- Manage any urological abnormality or underlying complicating factors, which is a strong recommendation 3
Clinical Decision Algorithm
- If the patient is systemically ill, hospitalize and use IV combination therapy for 14 days total duration 1, 3
- If prostatitis cannot be definitively excluded, treat for 14 days 2, 3
- If the patient has used fluoroquinolones in the last 6 months or local resistance is ≥10%, use an alternative agent 1, 3
